3.4.2.3 Intravascular access devices

Central venous catheters (CVCs) - which are are inserted ... Delete second 'are'.

Other vascular access devices: 'arterio-venous shunts' would be more appropriate than 'shunts'.

Other vascular access devices: Add midline catheters.

 

Risk factors for IVD-related BSI:

  • Prolonged placement of the device (NOT based on strong evidence)
  • Antibiotic use during catheterisation (What reference is this?)
PIVC stands for Peripheral Intravenous Catheter, not Peripherally inserted Catheter


Device selection: Consider the length of time that the catheter is likely to be in situ for... Remove the word ‘for’.
 
Site selection: - If a CVC is inserted into a jugular  vein, it should be resited into a subclavian vein or replaced with a PICC to reduce the risk fo infection .. Change 'fo' to 'of'.
 
Dressing: - If a patient is diaphoretic or if the site is bleeding or oozing, use gauze dressing until this is revolved ... Change 'revolved' to 'resolved'.
 
Device replacement: No recommendation for paediatrics?
 
Replacement of administration sets: - Change administration sets used to infuse herapin every 24 hours ... Change 'herapin' to 'heparin'.

Option 2: Replace a PIVC based on clinical indication.  Add a note to recognise that this is the real-world situation for any patient with difficult/limited venous access, IV insertion via ultrasound, paediatrics, neonates, etc.

A strategy of replacing IV cannula when a clinical indication for replacement is indentified  or the PIVC is no longer required (rather than routinely at 72 hours) may be considered ... Change 'indentified' to 'identified'.

There is demonstrated comprehensive documentation of insertion, maintanence and removal of PIVCs ... Change 'maintanence' to 'maintenance'.

Replacing a PIVC based on clinical indication can be cost saving and may reduce the discomfort for patients associated with regular replacement. ... And preserve veins… See the UK Vessel Health and Preservation literature
